It took me a while to find one that wasn't friendslocked... but you can see lydaclunas's version of the meme, here (http://www.livejournal.com/users/lydaclunas/115882.html#cutid1). There isn't an exact form for it, but most of the ones I've seen include some kind of physical/how I feel about my looks description, and various additions about personality, life experiences, surprising things about the person etc.
